Assessment Application Requirements
  1. 2 copies of Duly filled-up Application Form (c/o AC).
  2. 2 copies of Duly filled-up Self-assessment guide (c/o AC).
  3. Certificate of Training if Certificate of employment.
  4. PSA Birth Certificate if single, marriage contract if married.
  5. Passport size picture (4 pcs colored attire with collar, white background, formal pose and clear copy).
  6. COVID Certificate (TESDA Link).
  7. TESDA Processing fee: ₱ 35.00
